docs: sidebar refinements — Understand/Build reorder, Certified page, lexicon nav

Follow-up to PR #3. Refines the sidebar structure and fills gaps found during review.

Changes

Sidebar reordering

• Understand before Build — conceptual context before code
• "What are Hypercerts?" stays in Understand (conceptual), "How Hypercerts Work" stays in Build (data model with SDK references)
• Architecture moved up from bottom to after Build
• Roadmap moved into Reference (was a standalone item that visually bled into the section above)
• Final order: Understand → Build → Architecture → Tools → Reference

Certified page restored

• Brought back what-is-certified.md as a standalone page in Understand
• Content preserved word-for-word from PR Improve 'What is Certified?' page #1: why Certified exists (neutral identity provider, not tied to Bluesky), what you get, Bluesky/ATProto account callout, app.certified namespace

Account & Identity reframed

• Intro rewritten — no longer claims "you need an AT Protocol account to build with Hypercerts"
• Now framed as a post-Quickstart configuration guide: custom domains, app passwords, org accounts, recovery
• Added "Why Certified?" subsection and app.certified namespace section from PR Improve 'What is Certified?' page #1

9 lexicon pages added to sidebar

Individual lexicon pages existed on disk but were unreachable — not linked from the sidebar or their parent index pages:

• General Lexicons: Shared Definitions, Location
• Hypercerts Lexicons: Activity Claim, Contribution, Evidence, Measurement, Evaluation, Collection, Rights
