Update consent flow for reporting previous MMR doses so that:

• We only for ask for date and country of vaccination
• If country is part of the UK, ask if the vaccination was given to schedule (and infer date of vaccination from child’s date of birth), else ask for the date of vaccination
• If country is outside the UK, ask for the date of vaccination
