What is the change?
Add "Have you ever smoked?" and "What is your date of birth?" questions and handle service exists if participants do not meet the elligibility requirements?
Why are we making this change?
This is an initial spike into how we might provide a date model for questions and answers.
It also includes changes to add fully featured playwright acceptance tests
