This is usually a CORS issue. Did it work before and then stopped working? What reverse proxy do you have between karakeep and firefox?

It was working fine for several months and then stopped. No reverse proxy, just direct local.

Do you by any chance know how to share the console/network logs of this request from firefox? To know the kind of errors we're hitting?

I'm having the same issue with the addon, which started happening after I turned off my TrueNAS machine for a week. When I turned it back on, I updated every app, and the Karakeep Firefox addon stopped working with this same error. I tried reinstalling the addon and that didn't fix it. Karakeep itself works fine.

Not sure how to get logs for this on Firefox, as the console shows nothing when trying to configure the addon.

It's fixed now for me after giving the extension the "Access your data for all websites" permission on Firefox.
